Overview

Enhabit Home Health & Hospice is seeking a Registered Nurse (RN) for home health care in Las Cruces, New Mexico. This role emphasizes the coordination of patient care, managing visits, and communicating with all disciplines involved in patient treatment. Key responsibilities

  • Perform visits and coordinate client care
  • Serve as the point of contact for all care disciplines
  • Consult with physicians and provide patient care details
  • Perform accurate OASIS collection
  • Ensure medication profiles are current
  • Report lab values to physicians in a timely manner
  • Attend weekly case conferences and monthly case manager meetings

Required skills

  • Currently licensed as a Registered Nurse (RN) in the state of employment
  • Minimum of one year of clinical experience preferred
  • Knowledge and skill in current nursing practice
  • Valid state driver's license and automobile liability insurance
