Overview

Enhabit Home Health & Hospice is looking for a Registered Nurse (RN) to join their team in New Kent, Virginia. This role is vital in providing home-based patient care and involves coordination among various disciplines to ensure comprehensive care delivery. Key responsibilities

  • Perform home visits and coordinate client care
  • Act as the point of contact for all care disciplines
  • Consult with physicians and provide detailed patient care information
  • Ensure accurate OASIS collections and current medication profiles
  • Report lab values to physicians in a timely manner
  • Attend weekly case conferences and monthly case manager meetings

Required skills

  • Current RN license in the state of employment
  • Minimum of one year of clinical experience preferred
  • Knowledge and skill in current nursing practices
  • Valid state driver's license and automobile liability insurance
