Overview

Enhabit Home Health & Hospice is searching for a Registered Nurse (RN) for their Home Health team in Murfreesboro, Tennessee. This role focuses on providing top-quality patient care in a home-based setting, where the RN will be the main point of contact for all care disciplines and manage the coordination of client care. Key responsibilities

  • Perform client visits and coordinate care for patients
  • Serve as the point of contact for all disciplines involved in patient care
  • Consult with physicians regarding patient care details
  • Perform accurate OASIS collection
  • Maintain current medication profiles
  • Report lab values to physicians in a timely manner
  • Attend weekly case conferences and monthly case manager meetings

Required skills

  • Currently licensed as a Registered Nurse (RN) in the state of employment
  • Minimum of one year of clinical experience preferred
  • Knowledge and skill in current nursing practice
  • Valid state driver's license and automobile liability insurance
