Overview

Enhabit Home Health & Hospice is seeking a Registered Nurse (RN) for home health care in Franklin, Tennessee. This role involves coordinating patient care and acting as a key contact for all disciplines involved in providing care, all within a company recognized as one of the best places to work in the country.

Key responsibilities

  • Perform patient visits and coordinate care
  • Act as the primary contact for all disciplines involved in patient care
  • Consult with physicians regarding patient care details
  • Perform accurate OASIS collection
  • Ensure medication profiles are current
  • Report lab values to the physician in a timely manner
  • Attend weekly case conferences and monthly case manager meetings

Required skills

  • Licensed Registered Nurse (RN) in the state of employment
  • Minimum of one year of clinical experience
  • Knowledge and skill in current nursing practice
  • Valid state driver's license
  • Automobile liability insurance
