Overview

Professional Case Management is seeking an RN Case Manager to join their team in Hermiston, Oregon. This role allows for flexible scheduling while providing in-home care to former Nuclear Weapons Workers with chronic and terminal illnesses. The RN will conduct nursing visits, perform assessments, and supervise caregivers to enhance clients' health and quality of life.

Key responsibilities

  • Conduct in-home nursing visits for assigned clients
  • Perform physical assessments and ensure home safety
  • Oversee medication management and healthcare provisions
  • Coordinate and manage client care conferences
  • Assist in staffing coordination and scheduling
  • Participate in performance improvement initiatives
  • Collaborate with other personnel to improve services
  • Document clinical activities and maintain accurate records

Required skills

  • Registered Nurse (RN) license in Oregon
  • Two years of nursing experience, including home care
  • One year of supervisory or case management experience preferred
  • Current CPR certification
  • Ability to manage and evaluate care effectiveness
