Overview

Sanford Health is seeking a compassionate RN Care Manager for a PRN role in Thief River Falls, Minnesota. The position involves coordinating patient care, improving health outcomes, and providing individualized nursing care while enjoying scheduling flexibility and a positive team environment.

Key responsibilities

  • Utilize the nursing process to provide individualized nursing care
  • Develop and implement individualized care plans
  • Collect and relay patient information to providers
  • Assist with the coordination of patient care across the continuum
  • Collaborate with the inter-professional team and community partners
  • Manage disease registries and follow up with patients
  • Focus on health and wellness
  • Demonstrate competency in nursing practice

Required skills

  • Bachelor's degree in Nursing strongly preferred
  • Graduate from an accredited nursing program
  • Minimum of two years' clinical experience preferred
  • Unencumbered RN license
  • Basic Life Support (BLS) certification required
  • Valid driver's license
