Overview

The Referral Coordinator at ClareMedica Health Partners LLC plays a crucial role in enhancing the lives of seniors by scheduling and managing referral appointments. This position involves working directly with patients to facilitate their access to necessary specialist care and ensuring that all referral processes are completed efficiently and effectively.

Key responsibilities

  • Schedule and coordinate referral appointments with outside specialists
  • Maintain knowledge of health plan carrier requirements for referrals
  • Provide excellent customer service in person and over the phone
  • Document and track referral processes to promote patient safety
  • Assist patients with navigating healthcare-related issues
  • Act as a point of contact for patients and families regarding referrals
  • Ensure timely communication and reminders for scheduled appointments

Required skills

  • High school diploma or equivalent experience
  • Minimum of 1 year of experience in processing referrals or similar roles
  • Knowledge of HIPAA regulations and medical terminology
  • Exceptional communication, time management, and organizational skills
  • Proficiency in Microsoft Office Suite and other relevant software
  • Ability to problem solve, multitask, and work independently and in teams
  • Bilingual skills are a plus
  • Preferred experience with EMR systems and healthcare settings
