Overview

The Referral Coordinator at ClareMedica Health Partners plays a crucial role in coordinating and scheduling referral appointments for patients. This position requires strong communication and organizational skills to ensure seamless interactions between patients and healthcare providers, fostering a supportive environment for seniors in the community.

Key responsibilities

  • Schedule and coordinate referral appointments with outside specialists.
  • Maintain knowledge of health plan carrier requirements for referral authorizations.
  • Provide exceptional customer service to patients through face-to-face and phone interactions.
  • Assemble clinical background and referral needs information for specialists.
  • Ensure timely processing and tracking of referrals.
  • Assist patients in navigating healthcare systems and addressing financial or social barriers.
  • Communicate effectively with patients, families, and service providers.
  • Remind patients about appointments via mail or phone.
  • Perform miscellaneous administrative tasks as assigned.

Required skills

  • High school diploma or equivalent experience.
  • Minimum of 1 year experience processing referrals, MA, or front desk experience.
  • Knowledge of HIPAA regulations and patient confidentiality.
  • Exceptional oral and written communication skills.
  • Proficiency in Microsoft Office Suite (Word, PowerPoint, Outlook, Excel).
  • Ability to multitask and problem-solve effectively.
  • Detail-oriented with strong organizational skills.
  • Bilingual is a plus.
  • Healthcare experience preferred.
  • EMR system experience preferred.
