Overview

Adoration Home Health is seeking a passionate Physical Therapist to join their team in Jeffersonville, GA. This full-time position focuses on providing expert, patient-centered care in a home health setting, with an emphasis on independent work supported by a collaborative team.

Key responsibilities

  • Perform physical therapy evaluation and treatment of patients based on physician orders
  • Establish measurable goals and develop treatment plans for patient independence
  • Maintain communication among the treatment team, physician, patient, and families
  • Document services provided for proper billing
  • Supervise Physical Therapist Assistants and Aides in patient care activities

Required skills

  • Bachelor's Degree in Physical Therapy
  • At least 2 years of experience as a physical therapist
  • Valid state license to practice
  • Current CPR certification
  • Community/hospice experience preferred
