Overview

ChenMed is seeking Internal Medicine Physicians for their outpatient centers in Chicago, providing care primarily to seniors. The role emphasizes patient-centered care with a capped patient panel and no weekend or evening shifts, allowing for a focus on delivering value-based healthcare.

Key responsibilities

  • Manage a capped patient panel of up to 450 patients
  • Act as a health coach to help patients set and achieve health goals
  • Simplify and prioritize tasks to facilitate patient behavior change
  • Demonstrate accountability for patient outcomes and cost-effectiveness
  • Collaborate with healthcare professionals and participate in clinical meetings
  • Champion a positive culture of love, accountability, and passion in the center

Required skills

  • MD or DO in Internal Medicine, Family Medicine or Geriatrics
  • State Medical License for Illinois
  • Board certification in Internal Medicine, Family Medicine or Geriatrics (preferred)
  • Basic Life Support (BLS) certification
  • Current DEA number for controlled substances
