Overview

ChenMed is seeking Geriatricians to join their outpatient centers in Chicago. As part of America's largest geriatric primary care provider, you will have a capped patient panel and a supportive team, focusing on innovative, patient-centered care.

Key responsibilities

  • Manage a patient panel of up to 450 patients with accountability for outcomes
  • Act as a health coach, helping patients set and achieve health goals
  • Simplify care processes to make behavior change more actionable for patients
  • Collaborate with healthcare professionals to ensure compliance with guidelines
  • Participate in risk and quality management programs
  • Champion a culture of love, accountability, and passion within the team

Required skills

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty
  • State Medical License for Illinois
  • Board certification or eligibility in relevant specialty
  • Basic Life Support (BLS) certification
  • Current DEA number for schedule II-V controlled substances
