FOX Rehabilitation is seeking licensed Physical Therapists in Long Island, NY, to provide exceptional one-on-one care to older adults in their homes or senior living communities. This role emphasizes autonomy, flexibility, and meaningful patient outcomes through personalized treatment plans and collaboration with a multidisciplinary team.

Key responsibilities

  • Provide one-on-one physical therapy to older adults in various settings
  • Perform evaluations and develop individualized plans of care
  • Deliver evidence-based treatment to support patient wellness and mobility
  • Document effectively within the EMR system per regulatory standards
  • Collaborate with an interdisciplinary team of healthcare professionals
  • Educate patients and caregivers on safety and functional independence
  • Offer telehealth and virtual care as appropriate

Required skills

  • Valid Physical Therapy license in New York
  • Degree from an accredited Physical Therapy program
  • Basic computer and EMR proficiency
  • Current CPR certification
