Overview

FOX Rehabilitation is seeking licensed Physical Therapists in Oak Creek, WI, who are passionate about delivering exceptional one-on-one care to older adults. This role involves working in various settings such as private residences and senior living communities to improve patients' mobility, independence, and quality of life through a personalized treatment approach.

Key responsibilities

  • Provide one-on-one physical therapy to older adults in their homes and senior living communities
  • Perform evaluations and develop individualized plans of care
  • Deliver evidence-based treatment and therapy sessions
  • Document patient care within FOX's EMR system according to regulatory standards
  • Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists
  • Educate patients and caregivers on safety and functional independence
  • Utilize telehealth and virtual care when appropriate

Required skills

  • Valid Physical Therapy license in the state(s) of practice
  • Degree from an accredited Physical Therapy program
  • Basic computer and EMR proficiency
  • Current CPR certification
  • Ability to meet physical demands of in-home care, including lifting and patient transfers
