Overview

FOX Rehabilitation is seeking licensed Physical Therapists in Fairfax, VA, to provide one-on-one care to older adults in their homes and senior living communities. This role emphasizes autonomy, flexibility, and impactful patient outcomes, incorporating a Geriatric House Calls model that enhances mobility and independence.

Key responsibilities

  • Provide one-on-one physical therapy to older adults in various settings
  • Perform evaluations and develop individualized plans of care
  • Deliver evidence-based treatment sessions to support patient wellness
  • Document within the EMR system in accordance with regulatory standards
  • Collaborate with an interdisciplinary team of healthcare professionals
  • Educate patients, caregivers, and staff on safety and function
  • Offer telehealth and virtual care when appropriate

Required skills

  • Valid Physical Therapy license
  • Degree from an accredited Physical Therapy program
  • Basic computer and EMR proficiency
  • Current CPR certification
