Overview

FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Clinton, IA, to deliver exceptional one-on-one care to older adults in their homes and senior living communities. This role allows therapists to enjoy autonomy and flexibility while positively impacting patient mobility, independence, and quality of life through a Geriatric House Calls model.

Key responsibilities

  • Provide one-on-one physical therapy to older adults in various settings
  • Perform evaluations and develop individualized plans of care
  • Deliver evidence-based treatment to support patient wellness and mobility
  • Document effectively within FOX's EMR system
  • Collaborate with an interdisciplinary team of health professionals
  • Educate patients and caregivers to promote safety and function
  • Provide telehealth/virtual care when appropriate

Required skills

  • Valid Physical Therapy license in the state(s) of practice
  • Degree from an accredited Physical Therapy program
  • Basic computer and EMR proficiency
  • Current CPR certification
  • Experience with patient evaluation and treatment planning
  • Knowledge of evidence-based practice
