Job Description

Overview

The Pharmacist- Transitions of Care at L.E. Cox Medical Centers is responsible for providing pharmaceutical care services in clinic or hospital settings. This role involves designing, implementing, and monitoring patient pharmacotherapies while ensuring medication safety and effectiveness.

Key responsibilities

  • Provide pharmaceutical care services in assigned clinic or hospital areas
  • Design, implement, monitor, evaluate, and modify patient pharmacotherapies
  • Participate in collaborative and protocol-driven practice
  • Complete documentation for pharmacist activities
  • Support research activities in the department and clinic
  • Contribute to continuous improvement and patient safety programs
  • Engage in teaching experiences and special projects

Required skills

  • Graduation from an ACPE Accredited School of Pharmacy
  • Ph.D. or B.S. in Pharmacy
  • PGY1 pharmacy residency training or three years' outpatient experience preferred
  • PGY2 specialty residency in Ambulatory Care preferred
  • Three years' outpatient work experience if no pharmacy residency
  • Proficiency in direct patient care
  • Excellent presentation, verbal, and written communication skills
  • Ability to work independently and collaboratively
  • Familiarity with Microsoft Windows, Office, and other office equipment
  • Missouri Pharmacist license or eligibility
  • MTM/MTS certification
