Job Description

Overview

The Pharmacist- Transitions of Care at L.E. Cox Medical Centers is responsible for providing pharmaceutical care services in various clinical settings. Key duties include designing and monitoring patient pharmacotherapies, participating in collaborative practices, and engaging in research and continuous improvement efforts within the department.

Key responsibilities

  • Design and implement patient pharmacotherapies
  • Monitor and evaluate medication effectiveness and safety
  • Participate in collaborative and protocol-driven practices
  • Document pharmacist activities
  • Support research activities within the department
  • Participate in continuous improvement and patient safety programs
  • Engage in teaching experiences and special projects as needed

Required skills

  • Graduation from an ACPE Accredited School of Pharmacy
  • B.S. or Doctor of Pharmacy degree
  • Strong initiative and leadership skills
  • Independent and collaborative work capabilities
  • Proficiency in Microsoft Office and Windows
  • Excellent presentation, verbal, and written communication skills
  • MTM/MTS certification
  • Missouri Pharmacist license or eligibility for licensure
