Overview

Vitas Healthcare is seeking a Palliative Care Registered Nurse (RN) to join their comprehensive palliative care program in Livingston, New Jersey. This role focuses on providing individualized and coordinated care to patients and families in the community, with a Monday to Friday schedule and no on-call or weekend responsibilities.

Key responsibilities

  • Act as Case Manager and coordinate patient care for assigned caseload
  • Provide individualized coordination of care to patients and families
  • Complete comprehensive physical and psychosocial assessments to identify patient needs
  • Assist in accessing community services to improve overall health and well-being
  • Educate patients and families on disease processes and treatment options
  • Provide emotional support and counseling to patients and families
  • Evaluate patient recovery after treatment and use appropriate resources as needed
  • Encourage patient goal-setting and advance care planning
  • Build relationships with healthcare team members involved in patient care
  • Facilitate discharge planning to promote better patient outcomes
  • Protect patient privacy and confidentiality

Required skills

  • Minimum 2 years experience as a Registered Nurse
  • 1-2 years Case Management experience
  • 1 year of community experience (home health, rehab, hospice preferred)
  • Knowledge of interdisciplinary team concepts
  • Experience in patient education, planning, and management
  • Exceptional communication and customer service skills
  • Reliable transportation and current state driver's license
  • Bilingual a plus
  • Bachelor’s degree preferred
  • Current and valid license in the state
  • BLS certification required
