Overview

VITAS Healthcare is seeking a Palliative Care Registered Nurse (RN) to join their team in Lombard, Illinois. This role involves providing coordinated care to patients and families as part of a comprehensive Palliative Care Program, focusing on personalized and supportive healthcare solutions.

Key responsibilities

  • Coordinate patient care for an assigned caseload as a Case Manager.
  • Conduct comprehensive physical and psychosocial assessments of patients and families.
  • Ensure appropriate referrals to specialists and support adherence to treatment plans.
  • Educate patients and families about disease processes and treatment options.
  • Provide emotional support and counseling to enhance transitions and adherence to treatment.
  • Evaluate patient recovery post-treatment and utilize resources for improved outcomes.
  • Encourage patients in goal-setting and advance care planning.
  • Collaborate with healthcare teams to manage transitions of care.
  • Facilitate proactive discharge planning to reduce readmission rates.
  • Safeguard patient privacy and confidentiality.

Required skills

  • Minimum 2 years experience as Registered Nurse
  • 1-2 years Case Management experience
  • 1 year of community healthcare experience preferred
  • Knowledge of interdisciplinary team concept
  • Experience in patient education, planning, and management
  • Exceptional communication and customer service skills
  • Reliable transportation and current state driver's license
  • Bilingual skills are a plus
  • Bachelor's degree preferred
  • Current and Valid RN License
  • BLS certification required
