Job Description

Overview

The Outreach Specialist for the Khmer Community at Element Care is responsible for developing relationships and implementing outreach strategies to maximize referrals of older adults eligible for the PACE program. The role requires collaboration with internal teams and extensive community engagement to promote PACE services effectively.

Key responsibilities

  • Develop and implement a personal Annual Business Plan for outreach and referrals
  • Collaborate with Information and Referral department and Enrollment team to meet goals
  • Create relationships with referral sources and identify qualified referrals
  • Participate in community outreach activities like senior fairs and community events
  • Document outreach activities in Salesforce CRM promptly
  • Present PACE program benefits to various internal and external groups
  • Provide market intelligence to management
  • Seek professional development through training and coaching
  • Develop strong relationships with Center IDTs and staff
  • Perform additional duties as assigned

Required skills

  • Bachelor's degree preferred
  • 1-5 years of experience in older adult services
  • 1-3 years of sales experience in healthcare or group insurance
  • Strong customer service and sales skills
  • Understanding of marketing principles and practices
  • Strong business acumen and relationship-building skills
  • Excellent verbal and written communication skills
  • Proficient in MS Outlook, Word, Excel, PowerPoint, and CRM applications
  • Knowledge of Medicare, Medicaid, and insurance
  • Valid Massachusetts driver's license and vehicle for marketing activities
  • Ability to lift and carry up to 25 pounds
  • Fluency in Khmer
