Job Description

Overview

As an Occupational Therapist at CenterWell Home Health, you'll play a crucial role in helping patients regain strength, mobility, and independence in their homes. You'll deliver personalized rehabilitation care, focusing on functional improvement and empowering individuals to overcome physical limitations.

Key responsibilities

  • Assess and screen patient's daily living and work-related skills.
  • Develop therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic tests to evaluate patient function.
  • Confer with physicians and healthcare team members on Plan of Care.
  • Provide therapeutic treatment and instruction per physician orders.
  • Review and expand retraining programs according to behavioral goals.
  • Consult with vocational team members to implement vocational plans.
  • Report patient status and progress to relevant stakeholders.
  • Participate in care coordination and discharge planning.
  • Design community reintegration activities for clients.
  • Recommend and design adaptive equipment to improve client environments.
  • Supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance.
  • Document patient care observations and interventions accurately.

Required skills

  • Current and unrestricted Occupational Therapist licensure
  • Current CPR certification
  • Valid driver's license and reliable transportation
  • Minimum six months of occupational therapy experience
  • Home Health experience preferred
