Overview

U.S. Renal Care is seeking a Nurse Practitioner to provide comprehensive patient care for individuals with End-Stage Kidney Disease (ESKD) in Vallejo, California. The role includes patient education, care management, and collaboration with nephrologists and other healthcare professionals, with a focus on holistic patient outcomes and health management.

Key responsibilities

  • Conduct ESKD rounding and patient visits under the guidance of nephrologists
  • Manage holistic patient care including activation and treatment of co-morbid conditions
  • Educate patients on ESKD and manage disease-related visits
  • Collaborate with various healthcare teams for patient care coordination
  • Perform physical examinations and order diagnostic tests
  • Document patient medical information and maintain electronic records
  • Prescribe medications and adjust treatment plans as necessary
  • Promote preventative health care practices
  • Ensure adherence to company policies and customer service standards

Required skills

  • Current license to practice as an Advanced Practitioner
  • 2 years clinical work experience
  • Nephrology or dialysis experience preferred
  • Excellent written and verbal communication skills
  • Understanding of physician practice operations
