Overview

U.S. Renal Care is seeking a Nurse Practitioner in San Jose to provide comprehensive care for patients with End-Stage Kidney Disease (ESKD) and Chronic Kidney Disease (CKD). The role involves collaborating with nephrologists and healthcare teams, focusing on patient education, care coordination, and managing patients' health conditions.

Key responsibilities

  • Perform ESKD rounding, including limited and comprehensive visits
  • Manage holistic patient care for ESKD patients
  • Provide ESKD patient education and comprehensive disease management
  • Collaborate with nephrologists, dialysis centers, and healthcare teams
  • Conduct physical examinations and order diagnostic tests
  • Prescribe medications and adjust treatment plans as necessary
  • Document medical information and maintain electronic medical records
  • Educate patients and families on health issues and preventative care

Required skills

  • Current license to practice as an Advanced Practitioner
  • 2 years clinical work experience
  • Nephrology experience preferred
  • Excellent written and verbal communication skills
