Overview

U.S. Renal Care is seeking a Nurse Practitioner for the Oakland/San Leandro area, responsible for providing comprehensive care management for patients with end-stage kidney disease (ESKD) and chronic kidney disease (CKD). The role includes patient assessments, education, and collaboration with nephrologists and other healthcare professionals to ensure high-quality care and support.

Key responsibilities

  • Conduct ESKD rounding and perform comprehensive patient visits
  • Manage holistic patient care including activation, depression monitoring, and oversight of co-morbid medical issues
  • Provide patient education on ESKD and CKD
  • Collaborate with medical teams including nephrologists, dialysis centers, and discharge care coordination teams
  • Perform physical examinations and order diagnostic tests
  • Prescribe medications and document patient medical information
  • Educate patients and families on health problems and treatment plans
  • Promote GUEST customer service standards and maintain reliable attendance

Required skills

  • Current license to practice as an Advanced Practitioner
  • Specialty certification required
  • 2 years of clinical work experience
  • Nephrology or dialysis experience preferred
  • Excellent written and verbal communication skills
