Job Description

Overview

The Nurse Practitioner will work in the VBC Central Valley in Modesto, primarily focusing on managing patients with End-Stage Kidney Disease (ESKD) and Chronic Kidney Disease (CKD). The role involves comprehensive patient care including education, disease management, and collaboration with healthcare teams to ensure holistic patient outcomes.

Key responsibilities

  • Conduct ESKD rounds and comprehensive patient visits under nephrologist guidance
  • Manage holistic patient care including activation, depression oversight, and hospitalization risk
  • Educate patients on ESKD and CKD management
  • Collaborate with nephrologists, dialysis center teams, and other healthcare providers
  • Perform physical examinations and assess diagnostic tests
  • Prescribe medications and document medical information in electronic medical records
  • Encourage preventative health care and positive health behaviors
  • Maintain regular and reliable attendance

Required skills

  • Current license to practice as an Advanced Practitioner
  • Specialty certification
  • 2 years clinical work experience
  • Nephrology experience preferred
  • Excellent written and verbal communication skills
