Overview

Conviva Senior Primary Care is seeking a full-time Nurse Practitioner or Physician Assistant to join their clinic in Daytona Beach, Florida. The role involves delivering personalized, high-quality care as part of a collaborative team dedicated to improving the lives of senior patients.

Key responsibilities

  • Provide comprehensive care to seniors according to clinical standards
  • Collaborate daily with a care team to ensure coordinated, high-quality care
  • Manage referrals, hospital coordination, DME, and home health services
  • Document care with the support of quality-based coders
  • Participate in on-call rotation and support growth plans
  • Provide value-based care

Required skills

  • Master's degree in nursing or completion of a PA program with board certification
  • Active, unrestricted NP or PA license in Florida
  • Experience in value-based care, primary care, or Medicare Advantage
  • Active DEA license and ability to obtain Medicare/Medicaid provider numbers
  • Familiarity with HEDIS quality indicators and Medicare guidelines
