Job Description

Overview

AccentCare, Inc. is seeking a full-time Nurse Practitioner specializing in Palliative Care to support patients with chronic and terminal illnesses in Glendale, California. This role involves developing clinical care plans, performing assessments, and collaborating with healthcare teams to ensure optimal patient outcomes in a compassionate environment.

Key responsibilities

  • Develop and implement clinical plans of care for patients
  • Perform patient assessments and manage symptoms
  • Monitor patient progress and update medical team
  • Communicate with supervisory and attending physicians regarding care plans
  • Coordinate care with Palliative Care staff and outside agencies
  • Prescribe medication as permitted by state regulations
  • Participate in ongoing education programs for hospice-appropriate patients

Required skills

  • Graduated from an approved nursing school
  • One year experience as a Nurse Practitioner preferred
  • Licensed to practice as a Nurse Practitioner in California
  • Active DEA license
  • Valid CPR certification
  • Strong communication and coordination skills
  • Ability to travel to various patient locations
