Overview

AccentCare, Inc. is seeking a full-time Nurse Practitioner for Palliative Care in the Seaford, DE area. This role involves caring for patients with chronic and terminal illnesses within a collaborative and purpose-driven team that values excellence and compassion.

Key responsibilities

  • Develop and implement clinical plans of care for patients with chronic and terminal illnesses
  • Perform patient assessments and manage recommendations on symptom management
  • Monitor patient progress and communicate updates to the medical team
  • Coordinate care with attending physicians and other Palliative Care staff
  • Prescribe medications as permitted by state regulations
  • Participate in ongoing education programs and care for hospice-appropriate patients

Required skills

  • Graduated from an approved school of professional nursing
  • One year of Nurse Practitioner experience preferred
  • Active Nurse Practitioner license in the state of practice
  • Active DEA license
  • CPR certification preferred
  • Valid driver's license for travel to business locations
