Overview

AccentCare, Inc. is seeking a Nurse Practitioner for Palliative Care in Millsboro, Delaware. This full-time role involves providing care to patients with chronic and terminal illnesses, emphasizing a collaborative and supportive team environment.

Key responsibilities

  • Develop and implement clinical plans of care for patients
  • Perform patient assessments, history and physical evaluations
  • Monitor patient progress and communicate with healthcare team members
  • Initiate communication with attending physicians and other staff for optimal care coordination
  • Prescribe medication as permitted by state regulations
  • Participate in educational programs and continuity of care for hospice-appropriate patients

Required skills

  • Graduate from an approved nursing school
  • One year of experience as a Nurse Practitioner preferred
  • Licensed to practice as a Nurse Practitioner in Delaware
  • Active DEA license for prescribing
  • Valid CPR certification preferred
  • Ability to travel to various business locations
