Overview

Oak Street Health is seeking a Bilingual Nurse Practitioner to provide effective and equitable value-based primary care to Medicare adults, focusing on keeping them healthy and out of the hospital. In this role, you will work as part of a collaborative care team to deliver exceptional primary care, conduct office visits, and engage in population health management.

Key responsibilities

  • Provide exceptional primary care to adult patients
  • Conduct office visits for routine and acute health issues
  • Administer annual wellness visits and health risk assessments
  • Coordinate care with other providers, specialists, and testing facilities
  • Lead population health initiatives in collaboration with the care team
  • Educate patients on health conditions, care plans, and treatments
  • Conduct home visits as needed
  • Participate in promotional activities for Oak Street Health

Required skills

  • Master of Science in Nursing or equivalent NP program graduate
  • National certification as a Nurse Practitioner in relevant specialties
  • Active state Nurse Practitioner license
  • Active DEA license
  • Experience in primary care, internal medicine, geriatrics, or family medicine
  • Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments
  • Bilingual proficiency in Spanish
