Overview

Conviva Senior Primary Care is seeking a full-time Nurse Practitioner to join their clinic in Jacksonville, Florida. As part of a collaborative team comprising physicians, pharmacists, and specialists, the NP will focus on delivering personalized care to seniors, emphasizing physical, emotional, and social wellness.

Key responsibilities

  • Provide comprehensive care to seniors following clinical standards
  • Collaborate daily with the care team for coordinated, high-quality care
  • Manage referrals, hospital/SNF coordination, DME, and home health services
  • Document care with support from quality-based coders
  • Participate in on-call rotation and support growth plans at the center
  • Work on-site

Required skills

  • Master's degree in nursing with board certification
  • Active, unrestricted NP license in the state of practice
  • Experience in value-based care, primary care or Medicare Advantage settings
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain them
  • Familiarity with HEDIS quality indicators and Medicare guidelines
