Overview

As a Nurse Practitioner at Conviva Senior Primary Care in Lakeland, Florida, you will join a collaborative, multidisciplinary team dedicated to enhancing the quality of care for seniors. This role involves providing comprehensive care to patients aged 65 and older, focusing on their physical, emotional, and social wellness while adhering to clinical standards and compliance requirements.

Key responsibilities

  • Provide comprehensive care to seniors following clinical standards
  • Collaborate with the care team to ensure coordinated, high-quality care
  • Manage referrals, hospital/SNF coordination, DME, and home health services
  • Participate in daily huddles and document care with support from quality coders
  • Participate in on-call rotation and support growth initiatives at the center
  • Ensure compliance with all licensing and accreditation requirements

Required skills

  • Master's Degree as a nurse practitioner or physician assistant program completion
  • Active, unrestricted NP or PA license in the state of practice
  • Two years of experience in value-based care or geriatric or primary care
  • Experience managing adults aged 65+ with chronic conditions
  • Computer proficiency (EMR, email, etc.)
  • Experience in Medicare Advantage settings (preferred)
  • Active DEA license and Medicare/Medicaid provider numbers (preferred)
  • Familiarity with HEDIS quality indicators and Medicare guidelines (preferred)
