Overview

As a Nurse Practitioner at Conviva Senior Primary Care in Rockport, Texas, you will be part of a multidisciplinary team focused on delivering personalized and high-quality care to seniors. This role emphasizes a collaborative approach, ensuring that patients receive comprehensive and coordinated services in a supportive environment.

Key responsibilities

  • Provide comprehensive care to seniors following clinical standards.
  • Collaborate daily with the care team to ensure coordinated, high-quality care.
  • Manage referrals, hospital/SNF coordination, DME, and home health services.
  • Document care accurately and efficiently with the support of quality-based coders.
  • Participate in on-call rotation and support growth plans at the center.

Required skills

  • Master's Degree in Nursing or completion of a PA program with board certification.
  • Active, unrestricted NP or PA license in the state of practice.
  • Experience in value-based care, primary care or Medicare Advantage settings preferred.
  • Active DEA license or ability to obtain it.
  • Familiarity with HEDIS quality indicators and Medicare guidelines.
