Overview

Kindred at Home is seeking an LPN/LVN Intake Specialist in Temple, Texas, to enhance home health care services. This role involves interacting with referral sources and potential patients, assessing needs, and facilitating effective communication to ensure high-quality care is delivered to clients in their homes.

Key responsibilities

  • Receive and respond to incoming calls from referral sources or potential patients
  • Record call outcomes and follow up as necessary
  • Enter referral information in the system, including clinical information and service orders
  • Identify potential payer sources and document relevant information
  • Notify branch of referrals and maintain communication with referral sources
  • Generate sales correspondence and assist in sales/marketing activities

Required skills

  • LPN/LVN licensure in the state(s) of practice
  • Minimum of three years intake/clinical experience in home health
  • Strong knowledge of governmental home health agency regulations and Medicare policies
  • Proficiency in computer skills including Microsoft Office and data entry
  • Excellent negotiation, organization, decision-making, and communication skills
