Overview

Kindred at Home is seeking an LPN/LVN Intake Specialist in San Jose, California. The role focuses on providing compassionate care and excellent service through effective communication with referral sources and potential patients, ensuring their needs are met and appropriate services are identified.

Key responsibilities

  • Receive and respond to incoming calls from referral sources or potential patients
  • Identify patients' needs and determine the company's ability to meet them
  • Record outcomes of calls and track conversion ratios
  • Enter referral information into the system, including clinical information and service orders
  • Identify potential payer sources and document patient demographics and coverage availability
  • Notify the branch of referral status via email
  • Maintain working knowledge of services and community resources
  • Interact with referral sources to facilitate communication and resolve issues
  • Assist in obtaining ongoing authorizations for visits and document approvals
  • Generate sales correspondence and assist in marketing activities

Required skills

  • Graduation from an accredited school of nursing
  • Minimum of three years clinical experience in the area of specialty
  • LPN/LVN licensure in the state(s) of desired practice
  • Acute/homecare experience within the past two years
  • Strong knowledge of governmental home health agency regulations and Medicare policies
  • Working knowledge of personal computers and software, including Microsoft Office
  • Excellent negotiation, organization, decision-making, and communication skills
