Overview

Kindred at Home is seeking a compassionate and skilled LPN/LVN Intake Specialist in San Diego, California. The role involves receiving and responding to calls from referral sources, determining patient needs, and coordinating care services while ensuring compliance with regulatory requirements.

Key responsibilities

  • Receive and respond to incoming calls from referral sources or potential patients
  • Identify patient needs and determine the company's ability to meet them
  • Record call outcomes, make follow-up calls, and track conversion ratios
  • Enter referral information including clinical information and orders for services
  • Document demographic, clinical, and payer information and determine coverage availability
  • Notify branch of referral status via email
  • Develop and maintain knowledge of company services and community resources
  • Facilitate communication with referral sources and resolve problems
  • Obtain ongoing authorizations for visits and document them
  • Assist in sales and marketing activities

Required skills

  • Graduation from an accredited school of nursing
  • Minimum of three years clinical experience
  • Acute/homecare experience within the past two years
  • LPN/LVN licensure in the state(s) of desired practice
  • Strong knowledge of governmental home health agency regulations and Medicare policies
  • Working knowledge of personal computers, data entry, email retrieval, and Microsoft Office skills
  • Excellent negotiation, organization, decision-making, and communication skills
