Job Description

Overview

As an LPN/LVN Intake Specialist at Kindred at Home, you will play a critical role in delivering compassionate and high-quality care to patients in their homes. This position involves responding to referral calls, assessing patient needs, and coordinating with various stakeholders to ensure appropriate care services are provided.

Key responsibilities

  • Receive and respond to incoming calls from referral sources or potential patients
  • Record call outcomes and follow up as necessary
  • Track and report on conversion ratios
  • Enter referral information into the system, including clinical information and orders for services
  • Identify potential payer sources and document relevant information
  • Notify branch of referral status via email
  • Maintain knowledge of company services and community resources
  • Facilitate communications with referral sources and resolve problems
  • Obtain ongoing authorizations for visits as needed
  • Assist in sales and marketing activities

Required skills

  • Graduation from an accredited school of nursing
  • Minimum of three years clinical experience
  • LPN/LVN licensure in the respective state
  • Acute/homecare experience within the past two years
  • Strong knowledge of governmental home health agency regulations and Medicare policies
  • Excellent negotiation, organization, decision-making, and communication skills
  • Proficient in Microsoft Office and data entry
