Overview

Kindred at Home is seeking a compassionate LPN/LVN Intake Specialist in San Diego, California, to provide high-quality care and support for patients in their homes. The role involves assessing patient needs, managing referrals, and ensuring effective communication between patients, family members, and healthcare providers.

Key responsibilities

  • Receive and respond to incoming calls from referral sources or potential patients
  • Record call outcomes and track follow-up communications
  • Enter referral information and document clinical and payer details
  • Notify branch of referrals and maintain communication
  • Obtain ongoing authorizations for patient visits as necessary
  • Assist in sales and marketing activities related to referrals

Required skills

  • LPN/LVN licensure in the state of practice
  • Minimum of three years of clinical experience
  • Acute/homecare experience within the past two years
  • Strong knowledge of home health agency regulations and Medicare policies
  • Proficiency in Microsoft Office or equivalent software
  • Excellent negotiation, organization, decision-making, and communication skills
