Overview

Kindred at Home is seeking a Nurse/LVN/LPN for their Lakewood, Colorado office to provide indirect patient care supporting physicians who visit home-bound patients. This role involves coordinating care and managing nursing duties in an office setting, utilizing phone and EMR (Electronic Medical Records).

Key responsibilities

  • Coordinate care with Physicians and Nurse Practitioners
  • Perform all clinical duties such as labs, orders, and prescriptions
  • Monitor services and documentation in compliance with policies and Medicare guidelines
  • Facilitate communication with patients, primary providers, families, and caregivers
  • Report patient conditions to the Physician or NP
  • Assist in resolving patient issues and enhancing patient satisfaction

Required skills

  • Graduate of an accredited School of Practical Nursing
  • Current state license as a Licensed Practical Nurse
  • At least one year of experience as an LPN, preferably in home care
  • Strong organizational and detail-oriented skills
  • Knowledge of medical terminology, clinical applications, and lab values
  • Proficient in computer use, including Microsoft Office and EMR
  • Familiar with business office procedures and basic office equipment
  • Ability to operate a multi-line telephone system
