Kindred at Home is seeking a Nurse / LVN / LPN for a role in Lakewood, Colorado, within the Kindred House Calls division. This position involves providing in-direct patient care in an office setting while supporting physicians who visit home-bound patients for primary care. The role is focused on coordinating clinical duties, improving patient satisfaction, and ensuring adherence to medical guidelines.

Key responsibilities

  • Coordinate care with Physicians and Nurse Practitioners
  • Provide in-direct patient care using phone and EMR
  • Manage nursing duties including labs, orders, and prescriptions
  • Monitor services and documentation to meet guidelines
  • Facilitate communication and education for patients and families
  • Report patient condition to the assigned Physician or NP
  • Assist in resolving patient issues to enhance satisfaction

Required skills

  • Licensed Practical Nurse (LPN) credential
  • At least one year experience as an LPN, preferably in home care

  • Proficiency in Microsoft Office and EMR utilization
  • Detail-oriented and organized
  • Ability to operate multi-line telephone systems
