Overview

Kindred at Home is seeking a compassionate and dedicated LPN/LVN Intake Specialist to join their team in Daly City, California. As a leading provider of home health services, the role involves interacting with potential patients and referral sources to assess needs and ensure high-quality care delivery in a home environment.

Key responsibilities

  • Receive and respond to incoming calls from referral sources or potential patients
  • Record outcomes of calls and make follow-up calls as necessary
  • Enter referral information into the System including clinical information and orders for services
  • Identify potential payer sources and document necessary patient information
  • Notify branch of referrals as pending or ready to be staffed
  • Develop and maintain knowledge of services and resources provided by the Company
  • Interact with referral sources to facilitate communication and resolve issues
  • Obtain ongoing authorizations for visits based on clinical staff information
  • Assist in sales and marketing activities by generating correspondence and contacting referral sources

Required skills

  • Graduation from an accredited school of nursing
  • Minimum of three years clinical experience in specialty area
  • Acute or homecare experience within the past two years
  • LPN/LVN licensure in the state of practice
  • Strong knowledge of governmental home health agency regulations and Medicare policies
  • Proficient in Microsoft Office and data entry skills
  • Excellent negotiation, organization, decision-making, and communication skills
