Kindred at Home is seeking a compassionate LPN/LVN Intake Specialist in Daly City, California. This role involves interacting with patients and referral sources to gather and document information, assess needs, and coordinate care services. It emphasizes clinical excellence and superior service, contributing to the company's mission to provide quality home health care.

Key responsibilities

  • Respond to incoming calls from referral sources or potential patients to identify needs.
  • Record call outcomes and track conversion ratios.
  • Enter referral information into the system, including clinical information and orders for services.
  • Identify potential payer sources and determine coverage availability.
  • Notify branch of referrals and maintain communications with referral sources.
  • Obtain ongoing authorizations for visits and document authorization details.
  • Assist in sales and marketing activities.
  • Develop and maintain knowledge of services and community resources.

Required skills

  • Graduation from an accredited school of nursing
  • Minimum of three years clinical experience
  • Acute/homecare experience within the past two years
  • LPN/LVN licensure in the relevant state
  • Strong knowledge of governmental home health agency regulations and Medicare policies
  • Excellent negotiation, organization, decision-making, and communication skills
  • Working knowledge of computer systems and Microsoft Office
