Overview

The Medical Social Worker at Oak Street Health plays a vital role within the care team by addressing the psychosocial needs of patients, particularly those at higher risk. This position requires collaboration with various healthcare professionals to coordinate care, advocate for patients, and provide education and support to improve health outcomes and prevent hospitalizations.

Key responsibilities

  • Provide comprehensive assessments and care coordination for high-need patients.
  • Engage with patients and their support persons to identify healthcare goals and address barriers.
  • Maintain knowledge of community resources and make referrals as needed.
  • Navigate patient insurance benefits to optimize healthcare utilization.
  • Educate patients on care options, including advance care planning and end-of-life discussions.
  • Document interventions and patient status for care team visibility.
  • Respond to crises and prioritize urgent needs effectively.
  • Collaborate with care teams to develop creative solutions for patient issues.

Required skills

  • Master of Social Work degree from an accredited program
  • Minimally licensed in the state of employment
  • Fluency in Spanish, Polish, Russian, or other local languages as necessary
  • Proficient in Microsoft Office, GSuite, and electronic medical records
  • Experience with complex care coordination and population healthcare management
  • Minimum one year of medical social work experience
  • Ability to work in multidisciplinary teams
  • Strong problem-solving skills and positive attitude
