Overview

The Manager of Case Management at San Juan Regional Medical Center is responsible for leading the Case Management team to ensure high-quality patient care and efficient resource utilization during hospitalization. This role involves overseeing daily operations, supporting patients' movement through acute care, and ensuring compliance with discharge planning processes and reimbursement principles.

Key responsibilities

  • Provide direction and leadership to the Case Management team
  • Oversee daily case management operations
  • Facilitate safe patient movement through hospitalization
  • Develop annual budget in collaboration with the Director
  • Hire, orient, mentor, and conduct performance reviews for staff
  • Participate in organizational quality initiatives
  • Promote effective communication with post-hospital care agencies
  • Understand and articulate healthcare economics and payment systems
  • Implement SJRMC's Service Standards in all activities

Required skills

  • Registered Nurse License in New Mexico
  • Bachelor's Degree in healthcare or related field or ADN with 10+ years of experience
  • 5+ years of leadership experience in a hospital setting
  • Hospital Case Management experience
  • Knowledge of discharge planning and reimbursement principles
  • Assertive and diplomatic communication skills
  • Team-oriented mindset
  • Accredited Case Manager certification (ACM) within 2 years of hire
