Overview

The Manager of Case Management at San Juan Regional Medical Center provides leadership to the Case Management team, ensuring high-quality patient care and effective resource utilization during hospital stays. This role involves facilitating patient movement through hospitalization and overseeing daily operations within the case management department.

Key responsibilities

  • Provides direction and leadership to the Case Management team
  • Facilitates patient movement through hospitalization to post-acute services
  • Oversees daily case management operations
  • Develops annual budget in collaboration with the Director
  • Hires, orients, mentors, and performs staff performance reviews
  • Actively participates in quality initiatives and audits for quality assurance
  • Maintains relationships with post-hospital care agencies
  • Understanding of healthcare economics and public policy

Required skills

  • BSN or Bachelor's Degree in healthcare or related field
  • Registered Nurse License in New Mexico
  • 10+ years of healthcare experience, with 5+ in a leadership role
  • Hospital Case Management experience
  • Knowledge of discharge planning process and reimbursement principles
  • Assertive and diplomatic communication skills
  • Team-oriented approach
  • Accredited Case Manager certification (ACM) within 2 years of hire
  • Process improvement certification (preferred)
  • Certified Case Manager (CCM) (preferred)
