Overview

The LPN Health and Wellness Coordinator at The Village at Mill Landing is crucial for overseeing the coordination and delivery of personal care services to residents in assisted living. The role involves effective communication with staff, residents, families, and vendors to ensure high-quality care in a supportive and engaging environment.

Key responsibilities

  • Coordinate and deliver personal care services provided by staff to residents
  • Provide necessary information to direct care staff for safe and high-quality care
  • Triage resident issues and coordinate with providers, families, and vendors
  • Assist the Health and Wellness Director/Opal Manager as needed

Required skills

  • Excellent communication skills
  • Strong time management skills
  • Creative and flexible problem solving
  • Experience in Assisted Living
  • Geriatric knowledge with relevant work experience
  • Minimum of two years of supervisory experience preferred
