Overview

The Licensed Practical Nurse (LPN) at Avera Health is responsible for providing professional nursing care and assessments for residents in a long-term care setting. This role involves working as part of a multidisciplinary team, ensuring quality resident care, and participating in care planning processes.

Key responsibilities

  • Provide nursing care, assessment, and evaluations per scope of practice
  • Perform licensed practical nursing functions as directed by Registered Nurse
  • Complete assessments and apply nursing process to prioritize resident care
  • Administer medications safely and accurately according to physician's orders
  • Participate in care planning and implement care plans consistently
  • Communicate changes in resident conditions effectively to physicians
  • Assist with orientation of new staff and guide care team in meeting resident needs
  • Investigate incidents and address patient/family complaints
  • Facilitate conflict management while respecting diversity

Required skills

  • Licensed Practical Nurse (LPN) certification
  • Knowledge of nursing assessment and care planning
  • Proficient in medication administration, including Pyxis system
  • Effective communication and conflict management skills
  • Ability to work collaboratively in a team environment
