As a Physical Health Therapist working in home care, you will engage with patients in their own environments, allowing for greater flexibility and a more personalized approach to their health and wellness. Key ResponsibilitiesConduct thorough assessments to evaluate patients' physical health needs and develop individualized treatment plans aimed at improving their mobility and overall physical health.Utilize evidence-based techniques to deliver physical therapy interventions, including but not limited to therapeutic exercises, manual therapy, and modalities such as heat, cold, and electrical stimulation.Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes, documenting all sessions clearly and accurately in compliance with legal and ethical standards.Provide education to patients and their families regarding physical health issues, self-care strategies, and ongoing management of health conditions.Collaborate with a multidisciplinary team, including medical professionals and mental health therapists, to ensure comprehensive care that addresses both physical and psychological well-being.Participate in case conferences, contribute to team discussions, and engage in ongoing professional development activities to stay updated on the latest advancements in physical therapy. Patient Population / Client Focus As a Licensed Physical Health Therapist at Health Coast Behavioral Center, you will work with a diverse patient population, encompassing individuals recovering from surgery, those with chronic pain conditions, and patients with various physical disabilities. QualificationsA Bachelor's Degree in Physical Therapy from an accredited institution is required.Current licensure as a Physical Therapist in the state of California.Strong interpersonal and communication skills to effectively collaborate with patients, families, and interdisciplinary team members.In-depth knowledge of physical therapy practices and a commitment to evidence-based care.Demonstrated ability to perform comprehensive evaluations and develop effective treatment plans tailored to individual patient needs.
