Job Description

Overview

Communitas, Inc. is seeking an Intensive Family Support Coordinator in Wakefield, Massachusetts, to provide individualized, family-centered services for people with intellectual and developmental disabilities. This full-time role involves assessing family needs, developing support plans, and connecting families with community resources and services.

Key responsibilities

  • Conduct home visits to assess family needs and identify access to community supports.
  • Facilitate the development of a Family Support Plan to support individual growth and independence.
  • Assist families in accessing state and federal entitlement benefits.
  • Provide trauma-informed care and support families navigating mental health and behavioral challenges.
  • Collaborate with clinical and medical providers for coordinated care.
  • Facilitate the use of DDS flexible funds for short-term family needs.
  • Work with local hospitals and community organizations to identify support personnel.

Required skills

  • Master's degree in a human services-related field
  • Minimum of two years of experience providing direct support to families
  • Valid Driver's License and acceptable driving record
