Overview

This position at Mercy is for a Hospital-Community RN Liaison, focusing on Hospice, Home Health, and Home Infusion referrals in various care settings. The role emphasizes excellent customer service to healthcare professionals and patients, while ensuring timely discharges and continuity of care within the Mercy Health System.

Key responsibilities

  • Accept Hospice, Home Health, and Home Infusion referrals
  • Maintain an active presence in acute care and community settings
  • Deliver excellent customer service to discharge planners and medical professionals
  • Assist in facilitating timely patient discharges
  • Support continuity of care initiatives
  • Communicate services effectively to physicians and post-acute communities
  • Document nursing assessments and care plans in a timely manner

Required skills

  • Registered Nurse (RN) license
  • Minimum three years of nursing experience
  • Experience in Hospice and Home Health preferred
  • Experience in in-patient acute and community settings preferred
  • Excellent observation and nursing assessment skills
  • Strong communication and customer service skills
  • Basic computer skills and understanding of Electronic Medical Records
