Overview

The Hospice RN Team Manager at Vitas Healthcare is responsible for leading the Patient Care Team, supervising and coordinating team members to ensure continuity of care for patients from admission to bereavement. This role involves acting as a patient advocate and managing interactions with social service and healthcare providers.

Key responsibilities

  • Supervise, evaluate, and coordinate interdisciplinary team members
  • Ensure continuity of care for patients from admission to bereavement
  • Act as patient advocate and coordinator for healthcare providers
  • Maintain patient records throughout the care process

Required skills

  • Knowledge of primary medical and nursing care principles
  • Experience in oncologic nursing and palliative care
  • Understanding of interdisciplinary healthcare team dynamics
  • Experience with VITAS information system (Vx)
  • Two years of supervisory experience in healthcare
  • Current and valid R.N. License
