Job Description

Overview

The Hospice and Palliative Nurse Practitioner at Vitas Healthcare will provide consultation in palliative care, symptom management, and supportive care for seriously ill patients and their families. This role involves working closely with an interdisciplinary team to implement care plans and educate others on palliative care services.

Key responsibilities

  • Facilitate clarification of patient and family goals of care
  • Assist with diagnosis, treatment, and management of acute and chronic health conditions
  • Educate facility staff and the community on palliative care services
  • Attend and lead internal team meetings
  • Collaborate with physicians and care teams on plans of care
  • Maintain accurate patient records and documentation
  • Prescribe medications and order diagnostics as necessary
  • Communicate diagnostic findings to relevant parties
  • Supervise palliative care team members when applicable
  • Participate in on-call rotation as needed

Required skills

  • 2-5 years nursing experience, preferably in oncology, hospice, geriatrics, or intensive care
  • Knowledge of interdisciplinary team concepts
  • Experience in patient education and pain management
  • Strong communication skills, both verbal and written
  • Working knowledge of computers and various software packages, including Outlook, Excel, and Word
  • Master's degree in Nursing from an accredited university
