Overview

As a Home Care Physical Therapist (PT) at Mount Carmel Home Care, you'll leverage advanced technology and your clinical expertise to assist patients in regaining mobility and independence in their own homes. The role involves collaboration with a multidisciplinary team while enjoying the flexibility and autonomy that home care offers.

Key responsibilities

  • Provide one-on-one, relationship-driven care to patients
  • Collaborate with physicians, nurses, social workers, and home health aides
  • Utilize the Epic EMR System for streamlined charting and communication
  • Ensure consistent, predictable care schedules
  • Maintain knowledge of home health Conditions of Participation
  • Focus on delivering deep meaningful work without on-call requirements

Required skills

  • Bachelor's or Master's degree in Physical Therapy
  • Active Physical Therapist license in Ohio
  • Experience in home healthcare preferred
  • Working knowledge of home health Conditions of Participation
  • Experience with EMR systems
